NASA Reporting

NASA (National Aeronautics and Space Administration) has established an incident reporting system for aviation members such as pilots and ATC members to enhance safety in flight.
This is used as an anonymous way to enable pilots to report (observed or suspected) incidents to NASA so they have the chance to evaluate the incident and, if possible, find a solution to solve the problem causing the incident.
An example might be a runway incursion at a congested non-towered airport.
Maybe in this case the solution might be to install a tower at this airport to control/guide the flow of traffic and reduce the pilot's workload when determining the Airport/Pattern situation.
Incidents reported to the NASA will not be used in any enforcement action by the FAA or the administrator (14 CFR 91.25)
Note that accidents and criminal activities will be referred to the FAA and are not intended to be reported via this Form.
Also the incident reports remain anonymous. The reporting person fills out a slip with his contact information, which is used by the NASA to contact them in order to gain more information, if deemed necessary, but then will be timestamped and returned to the reporting person to
-provide the person with proof of filing the Incident Report
-ensure the anonymity of the reporter (no copy of the Identification slip remains at NASA)
